About Upper Peninsula Health Plan and Our Candyland Tree

Based in Marquette, MI, Upper Peninsula Health Plan (UPHP) is a managed care and provider service organization that has been serving residents of the Upper Peninsula for more than 20 years. We provide health coverage for individuals enrolled in Medicaid, the Healthy Michigan Plan, MIChild, Children’s Special Health Care Services, MI Health Link, and Medicare Advantage. But, we are more than just health insurance. In addition to serving our members, we support our community at large by partnering with providers, employers, and community groups who share in our passion for building a healthier Upper Peninsula.

  • A concession stand carload snack package that includes:
    • Four (4) small popcorns
    • Four (4) boxes of candy
    • Four (4) cans of pop
      • valid at any one of our upcoming 2023 movie events

Our tree will be displayed in the hospital atrium during the Festival of Trees.

RAFFLE TICKET INSTRUCTIONS

  • Please enter the raffle number above on your ticket if you would like to win this prize. One prize number must be written on every raffle ticket.
  • Raffle tickets can be safely dropped off at the front entrance of Schoolcraft Memorial Hospital.
  • All raffle tickets will be sorted by prize number and deposited in the correct prize bin by the Festival of Trees staff.
  • Prizes valued over $600 will be subject to taxes and a W-2G will be sent to the Internal Revenue Service.
  • Proceeds from this event will benefit the new Rehabilitation and Aquatic Therapy Center.
  • Need not be present to win at the time of drawing but required to arrange for pickup.  Raffle prizes, trees, and wreaths will not be delivered.
  • Must be 18+ to purchase a ticket and win. Please do not write the name of an underaged individual on a raffle ticket.

The care that was provided by The nurses (Cindy, Felicia Ruth) and staff was exceptional. The entire staff was very friendly and made you feel welcome. We received a call from Felicia the next day to check on my husband to check his pain level and do a follow up. The surgery area from check in to check out was well organized, clean, and up to date. Thank you to all of you for what you do so well and everything beyond your normal duties.
