We feel it is important to ensure our patient’s well being and peace of mind in the home setting. That’s why we work closely with our hospital,  Schoolcraft Memorial Hospital to help ease the transition from the hospital to home.  We also have great working relationships with other hospitals in state and out of state and can receive referrals for both our home health and hospice program from any licensed physician, hospital or long term care facility.

Home Health Care

Home Health services bring the expert care and attention Schoolcraft Memorial Hospital is known for right into your own home. Patients who no longer require hospitalization but still need skilled medical attention are perfect candidates for our HomeCare program. Our highly experienced registered nurses will come to your home to evaluate, treat and educate you on your specific medical care needs and management  A registered nurse is on call 24 hours per day 7 days a week. Home health care can be a less expensive and less stressful solution than an extended hospital or nursing home stay.

There are many medical services that are ideally suited for performing in home rather than enduring the stress of going to the hospital. Some of the care options we offer include:

We become very close to families in a short time. Following the death of their loved one, families often desire to give a tangible gift to the hospice team to show their appreciation. As individuals, we cannot accept gifts or money. Therefore, a special fund and the memorials we receive were created for this purpose.

How do I qualify for Home Health & Hospice?

To ensure our expert nurses are treating patients who truly need it, there are some qualifications that need to be met in order to be eligible for Home Health and Hospice. If you have any questions about qualification for either program, please call us at 906-341-3284 and we will be happy to answer your questions.

Home Health Requirements

  • A Physician’s Order.
  • A skilled need, for example nursing, physical therapy etc.
  • Be considered homebound, which means leaving home requires much forethought, planning and/or assistance.

Hospice Requirements

  • Physician certified terminal illness with a life expectancy of less than six months.
  • An available 24-hour caregiver.

Payment Information

We understand that seeking medical care can be a financially daunting experience. We strive to work with every patient to find a payment option that suits their needs. Medicare & most Medicare HMO insurances cover costs for qualifying individuals. Medicaid, Blue Cross Blue Shield, other insurances and private pay are payment options.

Community Service Clinics

Schoolcraft Memorial is also pleased to offer a range of patient services at our collection of Community Service Clinics. Our highly skilled staff will be able to assist you with a comprehensive selection of outpatient services.

Blood Pressure Screening

Monitoring blood pressure is an important part of elderly care and patients at risk of hypertension. Schoolcraft Memorial HomeCare offers complete blood pressure screening services in a range of convenient locations in Manistique and Germfask.

Foot care is an important part of maintaining a healthy lifestyle and a high quality of life. Schoolcraft Memorial offers a range of podiatric services for the local community provided by a highly qualified Licensed Nurse. Please note that appointments are required for all services to ensure appropriate care is provided to all our patients.

The care that was provided by The nurses (Cindy, Felicia Ruth) and staff was exceptional. The entire staff was very friendly and made you feel welcome. We received a call from Felicia the next day to check on my husband to check his pain level and do a follow up. The surgery area from check in to check out was well organized, clean, and up to date. Thank you to all of you for what you do so well and everything beyond your normal duties.

The facility was clean. My wait time was perfect, and the staff was very courteous.
I saw the ENT doc for first time, anxious he was going to cause me pain in a sore spot in my throat. He was so kind and knowledgeable as well as compassionate and professional, I was sincerely taken aback. He prepared me for everything he did AND he didn’t hurt me !!!
Everyone at Schoolcraft Hospital was very professional and explained the procedure step by step. The nurses to the anesthesiologist and then Dr Bambach kept me informed and explained the outcome after my colonoscopy. Thank you for such good care it was in and out of there in less than 3 hours.
