That’s why we work closely with our hospital,  Schoolcraft Memorial Hospital to help ease the transition from the hospital to home.  We also have great working relationships with other hospitals in state and out of state and can receive referrals for both our home health and hospice program from any licensed physician, hospital or long term care facility.

Home Health Care

Home Health services bring the expert care and attention Schoolcraft Memorial Hospital is known for right into your own home. Patients who no longer require hospitalization but still need skilled medical attention are perfect candidates for our HomeCare program. Our highly experienced registered nurses will come to your home to evaluate, treat and educate you on your specific medical care needs and management  A registered nurse is on call 24 hours per day 7 days a week. Home health care can be a less expensive and less stressful solution than an extended hospital or nursing home stay.

Hospice Care

At Schoolcraft Memorial Hospital, we understand that those requiring hospice care need compassion and thoughtfulness in all aspects of their days. That’s why we are proud to offer the finest hospice care available in the state, with the patient and not the illness as our top priority.

Our expertly trained team is dedicated to providing compassionate care and support services for patients and their loved ones. Our central goal is to assist patients with life limiting illnesses to continue their life with the support of various members of our team . With Schoolcraft Memorial Hospice care, our patients are free to live their lives with dignity, respect and purpose.

Hospice Donation Fund


We become very close to families in a short period of time. Families will often desire to give a tangible gift back to the hospice team to show their appreciation following the death of their loved one. As individuals, we cannot accept gifts or money, therefore a special fund was created for this as well as memorials we receive. Over the years the fund has been used to provide experiences for our patients and staff that might otherwise not have been affordable. Our hospice families have benefited from this fund as staff strive to be creative in supporting quality and enjoyment of life each day. Things like paying for travel expense so a child can return home to say goodbye; providing a salon appointment in the home; purchasing a holiday meal for a struggling family gathering for their last Christmas together; or purchasing specialized equipment to increase a person’s comfort are a few ways the fund has been used. It also enhances the ability of our staff: covering special training and conferences; purchasing bereavement and volunteer materials; and specialized tools to support work in the home. A donation to this fund can be made simply by clicking on the tab. And we continue to be grateful for the memorials we receive by mail as well.


Living alone can be a frightening prospect for an elderly relative or someone requiring frequent medical attention. At Schoolcraft Memorial HomeCare and Hospice, we ensure your loved ones always have the support they need with Lifeline. This personal emergency response system allows an individual to summon emergency care at any time of the day or night, on any day of the year.

One of our trained personnel will install a unit, teach you how to use it and test the distance the unit is operational. We ensure you are completely comfortable with the use of the machine so that should you ever need to use it, the care you need is just a button press away. Fees start at $50 for installation with $30 per month rental fee.

How do I qualify for Home Health & Hospice?

To ensure our expert nurses are treating patients who truly need it, there are some qualifications that need to be met in order to be eligible for Home Health and Hospice.

Home Health Requirements

  • A Physician’s Order.
  • A skilled need, for example nursing, physical therapy etc.
  • Be considered homebound, which means leaving home requires much forethought, planning and/or assistance.

Hospice Requirements

  • Physician certified terminal illness with a life expectancy of less than six months.
  • An available 24-hour caregiver.

Payment Information

We understand that seeking medical care can be a financially daunting experience. We strive to work with every patient to find a payment option that suits their needs. Medicare & most Medicare HMO insurances cover costs for qualifying individuals. Medicaid, Blue Cross Blue Shield, other insurances and private pay are payment options.

Schoolcraft Memorial is also pleased to offer a range of patient services at our collection of Community Service Clinics. Our highly skilled staff will be able to assist you with a comprehensive selection of outpatient services.

Foot Care Clinics

Foot care is an important part of maintaining a healthy lifestyle and a high quality of life. Schoolcraft Memorial offers a range of podiatric services for the local community provided by a highly qualified Licensed Nurse. Please note that appointments are required for all services to ensure appropriate care is provided to all our patients.

The care that was provided by The nurses (Cindy, Felicia Ruth) and staff was exceptional. The entire staff was very friendly and made you feel welcome. We received a call from Felicia the next day to check on my husband to check his pain level and do a follow up. The surgery area from check in to check out was well organized, clean, and up to date. Thank you to all of you for what you do so well and everything beyond your normal duties.
