Food as Medicine at Schoolcraft Memorial Hospital

Schoolcraft Memorial Hospital is now partnering with the Food As Medicine program in the Upper Peninsula. 

HOW CAN I QUALIFY FOR THE PROGRAM?

Patients must answer yes to 2 of the 3 questions to qualify:

  • Do you have a chronic health condition or at risk of developing?
    • Examples of Conditions: Hypertension, Diabetes, Pre-Diabetes, High Blood Pressure, Asthma, Cancer, COPD, Chronic Kidney or Liver Disease
  • Is there a food food insecurity in the home?
  • Is your income 200% or lower below the poverty line?

HOW DO I GET REFERRED TO THE PROGRAM?

  • Contact your provider for an application. Our patient navigator or the provider nurse can also get the application started. 
  • Your provider will fill out the Practitioner Referral Form.
  • Patients must sign the Participant Release.
  • Your referral form and release will be sent to the Patient Navigator which will then be forwarded to UPCAP.

HOW IT WORKS

  • Each voucher is worth $5 and you will receive a total of $15 per week.
  • Participants can utilize the program for a maximum of 2 years. 
  • Once you are enrolled in the program you will be mailed the vouchers monthly, $60.00 total.
  • Vouchers can be spent at any participating farmers markets, there are 23 locations across the UP between May – September. 
  • Vouchers will expire by September 30, 2026. You may spend vouchers each time you visit the market, they do not expire month to month within the market season. You may spend more one month and less in another, as long as you use them before September 30, 2026.
  • People enrolled through July 31, 2025 receive vouchers that may be spent until September 30, 2025. Even if you enroll late, you will still receive the full number of vouchers promised in the program.
  • If you enroll after August 1, 2025, you will not receive vouchers until the following spring.
  • Educational material is sent each month with the vouchers about recipes, different foods, and the storage of fruits and vegetables for later eating will be provided to all program participants.
